Default 94 Accord plug wire popped off

My son called me from college today complaining that his 94 Honda EX (180K miles) was running really rough. He's hard on it and doesn't maintain it well unfortunately.

So I popped the hood and one of the plug wires had come loose.

Question:
1. The wires show signs of aging...is it unusual for a wire to pop off the plug?
2. There was quite a lot of oil around the part of the wire that goes down inside the valve cover...but that would be expected wouldn't it?
3. Is there any way that pressure (blow by?) could build up inside the value cover and pop a plug out? Or am I just looking for trouble?

Once I stopped the engine, wiped off the plug wire and put it back on the car seemed to run fine.
