It's an auto tensioner to manual tensioner conversion. You replace the automatic tensioner on an H22A with a manual tensioner from an H23A1. This is done because the auto tensioner can wear out over time. Not to mention that they don't do as good of a job of putting tension on the belt as a manual one does. It's simply for added reliability, it doesn't allow you to rev any higher or anything like that.
