where do you feel the shake? In the steering wheel, in the seat, in the accelerator? Logic says its an engine misfire if the steering wheel isn't shaking and the problem goes away if you kick it into neutral so you can eliminate wheel balance or suspension.does the tachometer needle stay steady when this happens or does it start to bounce around. My gut tells me you have an ignition coil breaking down. that is a very common failure on the accords of that year. typically the problem is most evident when accelerating; you never feel it on deceleration which is what you're doing when you're in neutral.I'd check the coil; if its external you can feel it and it should be the same temperature as the surrounding components; if the coil is hot to the touch its breaking down. some accords of that era had internal coils (ignition coils inside the distributor) in that case you can't do the touch testhope that helps

additional possible: check the egr. valve and ports. disconnect the vacuum hose to the egr valve, plug it and test drive. no vibration? replace valve and clean the ports. Any Honda dealer can give you the information about cleaning the ports. It was either a service bulletin or a service news article about ten years ago.
