It can mess things up. There are some on the market that are balanced. Even the balanced ones aren't completely balanced, unless you balance them along with the entire rotation assembly. My friend had his oil pump destroyed because of an aftermarket pulley. On the other hand, there are a lot of people who use them with no problems at all.
