Depends on the year, the '98s-02s Accords are known to have transmission problems. The extended warranty on them is 109k miles or 7yrs 9 mos., whichever comes first. Costs around $3k to have the transmission replaced, although many are complaining about the replacements not being good either. I've seen a lot of complaints about the transmissions that have less than 100,000 miles on them.

I have a 95 civic with a 180,000 miles on it, with the original transmission still in it.

You don't say what year?I had an '85 Accord with 240,000 miles [387,000 kms] and never changed the fluid and never had a problem with it. It still ran like new when I got rid of it. No kidding!

I have a honda civic with over 250,000 miles on it so i can say yes they are reliable. I still get 32 mpg.
