Default 1990 Accord LX overheating

I'm driving a 1990 Accord LX with only 135k. For the past few days, after about 10 miles of driving the temp gauge starts climbing - first to halfway, then keeps rising until I blow heat (not fun since it's almost 100 outside). Blowing heat makes it hover around the 50% mark until I get it home.

This happens if I'm running a/c or not, if I'm doing freeway driving or street, night or day, rain or shine. The car is garaged.

I've checked:
1. fans and fan fuses (OK)
2. coolant levels (OK)
3. obvious coolant leaks (none, engine is dry and clean, no steam, gauge never gets to red marks cuz I rev up the heat, nothing comes out from underneath)
4. sweet white smoke from tailpipe? (OK, there is none)
5. wet spark plug? Nope (I just disconnected, pulled, and looked - car wasn't running and hadn't been run in hours)
6. oil condition - dark, gross, BUT I'm told by my mother (this is her spare car) it hasn't been changed in forever, and my cousin was driving it before me (makes about 18 months of no changing). I want to change it myself while engine is nice and "cold", but if I'm going to do that (and air filter and fluids and all that) I want to fix whatever else may be wrong (unless it's head gasket).

The past day of super-local, one-stop driving revealed a new issue - slightly rough engine running. No hesitation or power loss, just not the super-smooth running it's been doing for all of its almost 19 years.

Please don't say cracked head or head gasket ... Thermostat? Oil condition (thanks, cousin)?

I'd start by replacing the thermostat if it's over 4 years old.

Do you change the coolant every 2 years? If not it might have crud that needs flushing.
