Default I have a 1998 Honda accord ex what would cause it to isle up and down in park and reverse...

...but not in drive?

The most common cause is a bubble in the coolant driving the Fast Idle Thermo Valve nuts. There is a bleed nipple on top of the upper radiator hose housing on the engine (maybe on the thermostat housing on some models). With the engine cold, open that nipple 1/2 turn with a 12 mm socket and close it when a steady stream of coolant comes out. That needs to be done whenever the coolant is drained and refilled.
