I am looking for some information on replacing the driver side mirror
(2004 Accord, 4 dr, 4 cyl, EX); book or (preferably) web site. I would
also like to know how to order the exact part.

Are you going to need the glass only, or the whole assembly? If just the
mirror glass is broken, I'd go to my local glass shop. They usually can get
and install exact replacements cheaper than you could do it yourself, if you
went to the dealer for the part, and figured your labor was worth anything.
Less likely to fall out, as well.

autozone has and can order mirror glass as well.
