I have a 1998 Honda Civic EX Sedan with 206,700 miles,

I've never replaced the fuel filter, but there are no signs of rust or
hinderance in the fuel department.

Is is ok to replace just for routine maintenance?

if it ain't broke don't fix it (does the manual say replace it) ?

no. civic fuel filters are good for many more hundreds of thousands of
miles on standard pump gas. unless you have a starvation problem, leave
it alone.
